In order to better assist you in the preparation of your 2023 tax return, we’ve created this questionnaire of the information we will need. We prefer that you submit your response through our website or email info@iver.biz but you may print this page and fax or mail it.

1. We must have a current copy of you and your spouse’s driver’s license in order to file your tax return. Have you or your spouse received a new driver’s license since the filing of your last tax return? If yes, please provide a copy: you may upload a copy to info@iver.biz

2. If our office provided you with coupons to make advance quarterly (April/June/Sept/Jan) estimated tax payments: Did you make all estimated payments in the amounts listed on the coupons?

10. Did you receive ERC (Employee Retention Credit) funds? Did another company complete the application? If yes to both, we need to amend your tax return. (Due to fraud of more than $8 billion the IRS has suspended processing of new applications and initiated 252 criminal investigations. It is highly unlikely that you qualify for the credit unless you experienced significant revenue reduction as a result of the COVID pandemic.)

Additional Explanation:

Question 2: Estimated Payments would be defined as four quarterly payments, which would have been made in April, June, September and January. Not everyone has a requirement to make estimated payments.
Question 4: Most colleges electronically deliver the required Form 1098-T directly to the student’s school account. Please have the student log in to obtain the form.
