We prefer that you submit your response through your client portal, but you may print this page and fax or mail it.

1. We must have a current copy of you and your spouse's driver's license in order to file your tax return. Have you or your spouse received a new driver's license since the filing of your last tax return? If yes, please provide a copy.

Additional Explanation:

Question 2: Estimated Payments are four quarterly payments payable in April, June, September and January. Not everyone has a requirement to make estimated payments.
Question 4: Most colleges electronically deliver the required Form 1098-T directly to the student's school account. Please have the student log in to obtain the form
